A cluster randomized controlled trial of pro-active community case management for malaria in Chadiza District, Eastern Province, Zambia
Objectives
The objectives of the study are to:
- Assess whether weekly proactive integrated community case management (iCCM) (the intervention arm) year round, compared to passive iCCM with malaria case management for all ages (the current standard of care or control arm) reduces the prevalence of patent (by RDT) and subpatent (by PCR) parasitemia detected at household level by cross sectional surveys.
- Determine whether the confirmed malaria case incidence, detected by community health workers (CHWs) (either proactively or passively) and at health facilities, decreases over time in communities with weekly proactive iCCM compared to routine iCCM by comparing the difference in slope of confirmed malaria incidence (number of confirmed cases per population) in proactive vs routine arms using an interrupted time series analysis.

Rationale and Abstract
More than a decade after introduction of RDTs and ACTs, a minority of children with fever are tested for malaria and appropriately treated. Promoting active detection of symptomatic malaria infection on a weekly basis by community health workers may be an effective way to improve the uptake of malaria case management, with resultant impact on malaria morbidity and hopefully, malaria transmission. This study will be undertaken to demonstrate the feasibility and impact of weekly active detection of symptomatic malaria cases at the community level on malaria morbidity and transmission. These data will help inform future policy decisions.
