Using MDA to rapidly reduce malaria transmission in Ebola affected areas of Sierra Leone
Objectives
The objectives of the MDA were:
- To rapidly reduce malaria-related incidence and mortality rates by providing interval-MDA using ASAQ – targeting in 7 high burden districts and all populations (all ages including children under 5 and pregnant women);
- To rapidly reduce the number of febrile Ebola suspected episodes that would otherwise have required screening and isolation in the Ebola suspected holding centres to exclude Ebola as the cause of illnesses, and reduce the risk of Ebola transmission among malaria patients.
- To evaluate trends of malaria cases and Ebola alerts during the post-MDA weeks compared to the pre-MDA weeks in MDA- and non-MDA-chiefdoms.
Since the initial clinical symptoms of malaria and Ebola are similar, care-seeking behaviour by the communities in Sierra Leone diminished during the peak of the Ebola outbreak in fear of being held as Ebola suspected case or contracting Ebola in the Ebola holding centres. Many patients have resorted to self-treatment in the informal sector or doing nothing.
According to the “Guidance on temporary malaria control measures in Ebola-affected countries¨ issued by WHO in November 2014, Sierra Leone MoH implemented a Mass Drug Administration (MDA) campaign that targeted more than 2.6 million people. The MDA campaign comprised 2 rounds of Amodiaquine-Artesunate (first-line recommended treatment for P. falciparum in the country) separated by 2 weeks.
This MDA campaign in Sierra Leone included social mobilization (advocacy meetings with local stakeholders; press briefing, jingle slots on national radios and local FMs and TV panel discussions; posters and banners; megaphone-mounted vehicles and town criers), as well as impact assessment and medicines safety monitoring.
